You must be and reside as Suffolk County resident and/or be on Probation with Suffolk County Probation to gain access to the MADD Online VIP panel. To learn more and to be granted access for this Victim Impact Panel, fill out the inquiry form using the link below for review.

After submitting the form, individuals will be provided access to the Online VIP by the MADD VIP Support Team via email. Emails will be distributed weekly using the following schedule:

Tuesdays
When forms are submitted by Monday Evenings at 11:59pm

Fridays
When forms are submitted by Thursday Evenings at 11:59pm
